Changes Made
The form field that requested a “Social Security Number” has been changed to “Employee ID”. The following forms were updated to reflect that change:
- Dependent Tuition Waiver (PDF)
- Interinstitutional Waiver of Tuition (XLS)
- Waiver for Survivors of Deceased Employees (XLS)
